Patient non-adherence to medication use remains one of the problems in chronic disease management because it can hinder the achievement of optimal treatment outcomes. This study aims to analyze the level of patient adherence to medication use and identify its association with the success of chronic disease therapy based on findings from previous studies. This study employed a qualitative method with a literature review approach. Data were obtained from various relevant national and international journal articles, books, and institutional sources, and were analyzed descriptively through the stages of data reduction, categorization, comparison, and synthesis of findings. The results show that non-adherence is still found in a considerable proportion of patients with chronic diseases. A study in Indonesia reported a non-adherence rate of 57% among 11,408 patients, while a nationwide study involving 1,750 patients found that 45.3% of patients were non-adherent. The literature review also shows that better adherence is associated with better treatment outcomes, whereas non-adherence is associated with less optimal clinical outcomes, increased healthcare utilization, and a higher risk of mortality. The novelty of this study lies in synthesizing the association between medication adherence levels and treatment success across chronic diseases. These findings emphasize the importance of education, monitoring, communication, and continuous support from healthcare professionals to improve patient adherence.
